Wordpress plugin costumization (test/quiz plugin)

I have a wordpress site where I have a online test (quiz) that people can take. I created the test with this plugin: [login to view URL] I need some customization for that plugin. The test works well and the test results are shown after someone clicks on the button "Test absenden". What I want in addition to that: I want that the user who takes the test, has to fill out a form with his credentials. The form has to include the same fields as this form here: [login to view URL]://[login to view URL] (also with the newsletter application for the mailchimp newsletter system) After the user has filled out the form, the test results are shown to him. AND also an email with the same test results is getting sent to his email address and a blindcopy to the email address [login to view URL] This is the site with the test: [login to view URL]
