1. There is a google map on these 3 pages: [login to view URL] [login to view URL] [login to view URL] The map works but is not implemented correctly with the css, and looks broken. The map and the text box SHOULD look like it does on the demo site here: [login to view URL] and here: [login to view URL] 2. I would also like (647) 241-8041 to appear in the header bar, right centered in the middle of the page vertically between the logo and nav items. 3. The contact form needs to be updated. I have attached an image of the new fields. All fields should be required. 3b. Once the form is updated, I would like it placed in the footer area (where ACTION! Is now) on the other 4 pages.
