You want a desktop software that you need developed. I use latest JPA(hibernate) for data manipulation, connect to any known relational databases (Mysql, Postgresql, Ms Server or Oracle). You may also just want a desktop app that uses an excel file for storing its data. I do reports using Jasper reports or POI depending on requirement complexity.

Am here for all that.

I convert paper questionnaire to XLSForm so that the survey can be done using Android smart phone instead of paper. I also setup the server for collected data (Odk aggregate)
