The purpose of this module is to allow a user to submit a form that will capture various user details and allowing users to upload a media file that will include images and videos. The form content will be added to a Zendesk ticket while the upload file will be saved directly to AWS S3. The form will be contained within a HTML file that will manage the branding and headers.
