Build a registration form using jquery mobile / ajax

Need to create a well designed cross-platform registration form to collect user details. Must be mobile responsive. Must write to a table. NOTE 1: User enters number of children. Depending on number that user enters, the next entry fields will appear. If user enters 0, then no fields will appear for child age. If user enters 5, then 5 fields will appear so that we can capture the age of the child based on the number of children the user enter. NOTE 2: The age of child must record the year of birth into the the table based on the age that the user enters. NOTE 3: We must record an 'update_date_time' into the table to show the date and time the form was submitted. NOTE4: Favourite genres should be stored in 2 fields: fav_genre and hate_genre. Store an array in each based on user selections. Each selection should have a 3-letter value to represent the selection. NOTE5: When can you attend question should be recorded as a number value. Each checkbox has a value. Add the values together to give the final result. Checkbox 1 =1, then 2, 4, 8, 16, 32, 64,128,256. By doing this whatever value is stored we can understand exactly what options the user selected. NOTE 6: The car images should record the following in order: small / medium / sports / family NOTE 7: All fields are mandatory except Address 2, 'Would you like to hear more..." and the final 'car' question. NOTE 8: Form must follow the OWASP recommendations for security.
