Develop Checkout For a Ministry Store

An online ministry is opening an online store, which will have a substore. We are using Open Cart to build the stores! We need someone to code our account forms so that it gives the person a place to donate to and our checkout process! In the checkout process we need someone who can develop a place for the person to let us know what ministry or church they would like to donate to. Along with that place should be some vital information about the place. So basically it would be adding some textarea boxes in the checkout and account signup pages. We then would need some boxes removed such as company, telephone, fax, address_1 and shipping methods! Also needing removed from both sites the Wishlist and Newsletter info! This would be for both sites!
