Convert PSD into HTML/CSS for our site landing page

We have a PSD design for 2 pages - both use the same template/design with the only difference being a middle content box. It is effectively a 2 step form, step1 - select your city, step 2 - fill in your name, contact details, etc. When step 1 is completed, the box should slide to the left and step 2 should slide in from the right. When step 2 is completed, the user should be taken to our actual website. The user also has the option of performing a 3rd interaction - that is, he can click a login button and a login box should slide in replacing both steps. At the bottom of the page (footer area), we would like to have our partner's logos scrolling across the screen - any jQuery scrolling widget will do. We only need the design done and all functionality (ie: everything that interacts with our database) will be completed by our own team. The application itself is written in CakePHP 1.2 so you will need to have knowledge of this framework. We have our code setup on a SVN allowing you to easily work with our own developers and commit your changes.
