Design for Image to be printed onto front/back of square DVD

My company, PianoKeyz, is the pioneer in selling online piano lessons (videos) in the form of an online membership. I also will have piano DVD courses as well. 1) Create DVD sleeve image with front and back design, with the color scheme following PianoKeyz dot com (Note the attached template). The main title for the DVD series will be: Master The Piano I will probably also want the logo for PianoKeyz smaller and on there somewhere. I'm currently getting the logo for the site re-made, but I'll upload one that we can temporarily use now, and maybe replace in a few days. 2) I need one flat image of a DVD disc which I will use to actually print on the discs. The discs will contain the title Master The Piano and also a blank spot either next to or underneath in which I will put the specific course title such as "Jazz Improvisation" and I'll also put in text 'Disc 1' etc. That way the template is always the same, but I will add in titles as necessary when I actually print the discs. 3) Lastly, I need an image which will need to be 3D looking as the selling image that I actually post on my website for selling the DVD. Since the actual DVD disc will be slightly different because of the title for each course, I would imagine that what might be best in a sleeve image with the DVD barely coming out (so it doesn't show the series title). I will be using the site and company PsPrint to print and mail me the DVD sleeves. I have attached the necessary pdf which is their requirement. High pixel resolution is a must for great quality.
