WANTED: BootStrap Rockstar Front-end Designer for a SaaS service -- 2

Hello, How are you? I hope this finds you well. I'm looking for a Bootstrap Rockstar to design a fabulous form by enhancing an existing web form for an upcoming SaaS service. I'm looking for an A+ quality designer/developer, and you must produce work with precision and high quality. I do not have the time to train nor the patience for losers and time-wasters. This is a serious project, and I'm looking to hire the right person ASAP. In the attached zip file you will find three files where [login to view URL] is how the existing form looks like and needs to be converted to a user-friendly interface that uses panels/accordions similar to [login to view URL] and FB2.jpg. You can use the basic Bootstrap V3 design template. Once you complete the form, I will go ahead and hire you for completion of the rest of the pages. The budget value placed here is just a placeholder. Actual budget is pretty flexible depending upon the quality and the turnaround time. I'm looking to have this entire form done by the end of this week or early next week at the latest. Thanks!
