Fix and Add to AJAX/JScript Code(repost)

Hello I need you to fix and add to an ajax/jscript from. You need to make the form do the following: Selection box - Instances names, and value is the id. Selection box - Bosses that is only in the above instance Selection box - Level only available to the above boss Textfield - Item name Now it needs to validate makes sure all fields are entered, if not at the top of the form will show an error - "error: please enter item name" etc - The form also need to check and make sure there is not another item name already exist in the database just use an array ... i will change this. This must not be case sensitive... so like Item 1 will be the same as item 1. It will also convert the item name first letter to a capital so if i enter - item one of two it will convert to Item One Of Two. If all ok and the image captcha is correct, then ajax will submit the form and where it would say error, now says thankyou This is all, make the jscript work and job complete. Thanks
