Web user experience: 1: person will read information 2: person will than choose their service 3: that will bring up a web form. The web form will be filled out by them and submitted. 4: next page will have calender to schedule an appointment. (must be live) 5: next page will be payment with shopping cart and additional things to be added to shopping cart. (preferably use paypal as cc processing company but not required) 6: confirmation page with appointment time service requested Notes: must also be able to have one way web video chat at secured web address if required by web user. must include complete site design only logo will be supplied by me. Once information is submitted the information must be placed into a data base and emailed to a recipient. The information from the web form will also be placed onto a pre-written document and emailed as well. The documents and web forms must be editable by me "the administrator easily" website will include several hundred different web forms. (you only need to make one and make easily editable for the administrator to make the others) need a fully functional website with art work. If you can not do art work let me know and I will submit another request for artwork alone. The appointment is a video conference where the user will see me and get to video chat. They must see my face on the screen through a secure web page whos web address must change. if the user does not have a microphone, they can do it over the phone but still must be able to see the video. The calender must be live so that when someone makes an appointment that time spot is taken and can not be taken again. this can only happen after payment. ## Deliverables 1) Complete and fully-functional working program(s) in executable form as well as complete source code of all work done. 2) Deliverables must be in ready-to-run condition, as follows (depending on the nature of the deliverables): a) For web sites or other server-side deliverables intended to only ever exist in one place in the Buyer's environment--Deliverables must be installed by the Seller in ready-to-run condition in the Buyer's environment. b) For all others including desktop software or software the buyer intends to distribute: A software installation package that will install the software in ready-to-run condition on the platform(s) specified in this bid request. 3) All deliverables will be considered "work made for hire" under U.S. Copyright law. Buyer will receive exclusive and complete copyrights to all work purchased.
