5-steps web form

I need a 5-steps web form with the following requirements: - Form data could be personal data (name, address, etc.), questions or social media actions (e.g.) like, follow - Data will be stored in mySql database - Captcha validation - Every step of the form has to be saved. Even when a user doesn't fill the whole form (browser closing, for example), partial data should be stored. - Input data for forms will be defined in a XML file. An XML entry for an input will have several attributes like database mapping field, validations needed for that input and so on. - Jquery data validation in each step. Validations will be performed before passing to next step) - Simple html mockup must be provided - Some kind of configuration setup to allow less than 5 steps.
