real estate search form

I need a real estate search form built. I would like for it to be similar to the search form on templatemonster site. I will provide the fields and details. There will be aprox 14 fields, i.e. City, County, Bedrooms, Etc... and the search button. I will provide the form get method to you as my database is already built. with my project, I want the check box fields to pop in a lightbox...
