Website design with data entry functionality - utilizing mysql database

Need a programmer to create a website with customer side and backend functionality. Customer will have login page, selection page & data entry page. Pages will inlcude one specific photograph that the customer has to identify and enter information about. All of this information will be saved to the database. Specific passcodes will take customers to specific pages. There will be 3 page designs but each passcode will have a number of different pages depending on the number of photographs. The backend will be for admin access and user interface is less important. Functionality of the backend and information we need in the database will be provided. We will need to upload a csv file containg database information. Information also contains a .jpg image file name and we will need to upload the matching .jpg file with the csv file. This information will be displayed on pages and additional data entry will need to be collected and saved to the database. Additional more detailed information will be given to programmers who are interested in the project.
