Prior to bidding on this project, I need to know what your plan is for doing it. Also, let me know if you need pages designed before, during, or after your programming/coding.

• PHP and MYSQL (must be custom clean coded programming or convince me that a great program will work and show me that program so I can demo it) Must show examples of prior work

• Wysiwig and simple to use CMS throughout site

• Need simple autoresponder which keeps members emails and name in a database and I can send out messages

Static text sections- from Microsoft word documents need to be placed into the appropriate sections of the site. Need wysiwyg editor in order to easily make changes to these pages:

Sections

1) Board of Directors (I can update their Profile and picture-less than 10 of these simple profiles with persons small photo

2) Contact Us form (fill out basic contact form that sends information to my e-mail address)

3) Login (username/password/forgot password) (Logging in allows members to go to predetermined member's only areas in the site)

4) Articles (Part of the vbulletin suite. Just needs to be implemented into. Design will come from the designer after you install this

5) Hfpa Merchandise (Just provide one simple link to a site)

6) Trainer Profiles and zip code locator (Members can upload specified profile information including their picture so everyone can see it. I/site administrator can edit and delete profiles easily and ultimately must approve the profile posting before it is accepted. Anyone visiting site can view trainers located within 5/10/25 miles from the zipcode they enter.

7) Our Partner Links (Ability to place their links on my site) pg. 40

8) Affiliate partners Program- ability to place my banner on their site and pay them as affiliates with a special code. Must have ability to give them a code which they can use to provide all of their customers with a 10% discount. I also must be able to provide people who do not have a website with an affiliate code which they can give out to their customers. must be able to pay affiliate through paypal or by check

9) Gold or Platinum Membership and Recertification (different priveledges between gold and platinum membership. (directed to paypal and access to appropriate member priveledges areas once paid) I must be able to manually approve people who want to be members when they pay by check

10) Exam (100 question automatically graded when submit button at the end is pressed multiple choice and true/false exam must not)

When submit button is pressed the person will see their score

11) Those who pass the test with a specified grade of 80% or higher become certified. I/administrator am automatically notified when someone passes the test with a score of 80% or higher. An 80% or higher score allows them to become certified and this will notify me to print out certification, wallet sized certification, and letter of distinction with certification # on it, date of certification. (once they are certified, I must be emailed with the members automatically given certification number and get their information so I can place in their information and print/reprint their certificates)

After someone completes and passes the exam, I need to be made aware and automatically sent an email notification so that I can easily prepare and print these (3) forms. Must be in a print ready format and correctly sized for printouts. I can easily place the persons information on the documents. (name, date the exam was taken, certification number which goes by order and number is automatically sent to me in an email.) I must be able to print these out quickly and easily.

• Once a member passes a test, again I (the administrator) will be notified and they will be into a “certified members database.” I will be able to print materials for these certified database members.

Note: The certification numbers are automatically given to them and sent to me starting with 1500 and going up to 1501, 1502, [login to view URL] members have a specific certified membership number for the printable certificate, wallet sized card, and letter of distinction . (Spaces in these documents can be filled out and then printed by me. The fillable/blank form sections are shown on the sample certificate, sample wallet sized card, and letter of distinction. Programmer codes so that in the specific coordinates user information like first and last name, date test passed, expiration date. From this another image is created with the information on it. It can be printed. Generated by coordinates. (Less costly approach than the PDF approach???)

If cost is similar, use the PDF approach. (let me know what the cost differences are to prepare it both ways. (3) different forms to print. All Members are placed in one database and certified members also get placed into a 2nd database.

13) Home Fitness Rolodex (this is simply a contact list of a businesses address, phone number, email address, and contact person in list form. I/site administrator must be able to add, edit, delete contacts) paginated format

14) Recertification pg.74-75-simple link to paypal to pay a recertification fee and become a member for 2 years

-Once the site is build I need to have instruction on how to use the different sections of the site. Site parts need to go onto my server at godaddy. Will give FTP instructions and administrator permissions.
