Enhancements for Magento Flat Rate shipping module

The ultimate goal is to have a shipping module with fixed rate, that would also allow customers to choose from a list (in the form of drop-down) of available destination points. These points - are something like luggage storage boxes spread ed across the country, so it is possible to deliver to one of those boxes which is the closest one to customer point of residence. From administration point of view this does not need to be something complex like a separate interface for adding each individual pick-up point, as the only information required - is a pick-up point name. Basically, this can be done in the form of a text-area, that will be populated with a vertical list of entries. However, if there is a better way of doing this (for example using Magento's built-in functionality) and cost difference is not too large, lease feel free to offer your way of doing this! In the checkout process, this shipping method will be listed along with any other active methods and choosing it should result in the appearing of a drop-down right beneath, where customer should choose preferable pick-up point. Choosing a value from drop-down should be mandatory! Chosen value should be added to all relevant places, like order information in admin, My Account, transactional emails... in other words everywhere, where shipping methods is normally stated. The way I see it, all of the above said can be done as the addition to standard Flat Rate shipping method included in Magento default installation. It is also preferable to use Flat Rate as a starting point, as all setting present in that module should be available in the resulting module as well.
