Site for IQ testing (example in detail description)

I need the site similar to this one: [login to view URL] but with BlamAds content locker as payment method (simple JavaScript code that I have, which pastes into page code and looks like this: [login to view URL]). Home page should have nice and professional design. I'll provide you pictures for answers to questions and they need to be processed and fitted into design. Answers should be collected by PHP script, and final result would be generated over two tables for evaluating IQ quotient. After finished test, if user likes page on Facebook, he needs to complete one offer on content locker and if not, he needs to complete two offers (redirect user on page depending of sharing on FB). User should have possibility to download results in PDF and share them on Facebook, but page should be locked by content locker. After completing offers, page unlocks.
