419648 Custom Web Form (PHP/mySQL)

REQUIREMENTS: 1) convert Photoshop PSD (to be provided) to W3C validated XHTML/CSS web form 2) take Excel spreadsheet > convert to mySQL DB 3) each user will access the web form using a unique code sent in an email message (i.e. [login to view URL]) 4) form should display using information from mySQL DB using the UID passed through the URL as the key (i.e. this will be the "Primary Contact") 5) the form should ALSO display records where the BrokerNumber field matches the same BrokerNumber of the PrimaryContact UID passed through the URL (these are Secondary Contact user records) 6) the form should allow the DB to be updated (Add, Edit, Delete the Secondary Contact user records) 7) the form should enforce strict security rules: (a) users maniupulating the UID passed through the URL should be redirected to a Custom Error Page (content/design to be provided) (b) all standard security protocols for preventing SQL injection etc should be employed 8) any errors on the form should present appropriate error messaging (to be provided) 9) on successful update, the form should present a confirmation page (content/design to be provided) with the information that was updated 10) on successful update the form should send an HTML formatted email message to the Primary Contact email address with a confirmation of data that was updated * site should use AJAX-like controls for Add/Edit/Delete user functions (i.e. page should NOT refresh each time info is updated) SAMPLE EXCEL FILE STRUCTURE BrokerNumber | BrokerName | BrokerTitle | PrimaryContactYesNoField | BrokerEmailAddress | OtherEmailAddress | OtherYesNoField | LastUpdatedTimeStamp |
