Submit Form corrections/ PHP MySql

ok basically I have 3 different forms and the submit information to a mysql database Form 1 I am using a small script to send out an email after the submit button is clicked the email goes out but I want some of the info that’s posted on the form that was pulled from the database to go with it Validations do not work Form 2 the form is designed nicely but I need it to be formatted a bit to fit the window same as above some of the info that’s being pulled from the database needs to be also sent out with the email Now the third issue with this form is this: The form is basically an applications and this one has a co applicant on it, when I submit the form and check in the admin, it shows me the co applicants info on the main persons info place and the first person info is gone, and the place where it suppose to pull in the first person is blank, this might be a database things as well (missing stuff) Form 3 this form just needs to be formatted to fit the window
