Prototype website development

I’m looking to valid a business idea by driving traffic to a website and measuring conversion rates. I’m looking for someone to put together a 7 page website, 6 pages will be the same template with different text. The home page needs to have a simple calculator, users enter the quantity of boxes they require and the monthly total is displayed. When users click ‘book now’ they are taken to a google form informing that we are launching very soon and inviting them to leave their email address. ‘Book now’ clicks need to be measured as ‘Goals’ in google analytics, I’d also like to collect the ‘monthly total’ value on these clicks (just to a CSV file is fine). I’d like the prototype to look reasonably professional so you’ll need to create PSDs or adapt a theme. I’m not looking for a full on design job, this is a prototype for feedback, not the full site but it does need to look somewhat appealing. I will provide all the copy for the pages, page maps and a URL. I would like to get this up quickly, so please give me some idea of your timings, availability, hourly rate or project cost and a link to some of your work. I’m not necessarily looking for the prototype designer/developer to take the project forward beyond the prototype but if you do it’s going to require some or all of the following: Phase 2 Assuming the Prototype proves the business case we will then look to build the site out. We will develop a brand identity and redesign the site. The full site will require integration with a number of different APIs (subject to evaluation we arent married to any of these).
