Video Intro for online Yoga Show

I'm interested in getting a short intro for an online Yoga show. I envision the intro to be approximately 10 -15 seconds in length. I would be able to provide photos of the instructor, an eps logo, a royalty free audio track and if needed I have hours of footage of the instructor in HD (which I would purge to a couple of quick 3sec clips). The colours would probably be green (to coincide with the website). I'm looking for a creative intro to help with the launch of the new Vidcast. I work in Premier Pro CS4 so an HD MPEG/MOV/Quicktime/AVI would be acceptable. I appreciate your time and look forward to the replies. The last intro I posted on this site was awesome. Cheers Tim
