150057 simple .net form windows app

I need a simple app that will let users fill out a short form and store the data in a local database. When user launches app -a screen will display showing all entries they've made previously -user can sort by word or category (see below) -user can open an entry by double-click for editing -user can create new entry with "add entry" button -user can delete entry by click/highlight and "delete entry" button If Create entry is clicked: -launches a form -form contains ::category combo box (user can enter new category or select from previous) There will be a default category of General which cannot be deleted or overwritten ::private - public radio selector group (value = 1 or 0) ::text box (user enters word or phrase) ::checkbox (value = 1 or 0) ::text area (user enters word or phrase 1 per line) ::update/apply/save/close (user should be able to edit these later)
