MsgBox in Jquery add Select option

I need to add select field to Jquery MsgBox script, it has been added manually but does not work wheen passing data to php and vice versa. I think small addition need to be done in script. This job is of 5min for pro jquery programmer. I by myself are noob wheen jquery on table. I will send MsgBox js file at end.
