542151 Need Member Registration Form

Need a form that has login for existing users on the left side and register for new users on the right side. The existing users when they login will load a specific webpage on the [login to view URL] registration page they will register with basic information and they will select "7day trial" or "Purchase Software" if they select the trail then once they complete the registration the page will place a token on their computer to disable access and ask them to register and telling them their trial has expired. they can register and it will process their payment with paypal pro and activate their username and password. The User can then login with their username and password. Their needs to be a forgot username/password function. All info needs to be stored on a database that can be accessed through a simple admin interface where the admin can see all registered users contact info and payment process receipts including email and export emails in csv file. The admin should also beable to deactivate license from admin panel.
