Web/Graphic designer wanted to re-design online tax form

I am looking for a web/graphic designer to re-design an online tax form (currently under development and nearing completion). The form consists of about 10 pages which contain various questions relating to tax with answers in the form of text boxes, tick boxes and drop down menus. Also some tables with several columns of headings with rows of textboxes below for answers and option to add additional rows. Some of the tables also contain mathematical formulas. The form also has some sections which change dynamically depending on answers given to specific questions, for example, if specific question is answered in the positive then an additional question/table is added. As mentioned above, the form is currently being developed (by another developer) and they are close to completion. It is working OK but the current developer is not a designer so it has not been designed well and needs to be re-designed and tidied up to look more professional prior to launching. I believe the form has been created in PHP language and also has some Jquery or AJAX elements. You will need to be familiar with these programming languages in order to take on this project. The developer has used a stock image page header at the top of the form which will need to be replaced with a custom designed header with our company logo and details. The pages which are all fairly basic in their design will need to be improved with better fonts, buttons and tidying up the layout and fixing any alignment issues. This job is urgent as I am launching the site very soon so I am looking to get the design work completed in about 1 week. Please only apply if you can work within this timeframe.
